Please fill out all forms in clear block letters in black ink.

	First, fill out the Request for Quotation (Overseas Moves Only) Form in its entirety. This form includes a Cumulative Form/Inventory List for Insurance. It will give us an idea how much your valuables are worth. In accordance with this list we shall prepare your insurance quote . Please note we provide this form for your convenience. We do not require you to use our form. If you'd like, you can format one of your own.
	It is important to remember that when you state the value of your belongings for insurance you must insure them for the full replacement value at destination. Under insurance may result in repair or replacement cost not being paid in full.
	If you are shipping a motorized vehicle of any kind, you are required to fill out the required form Application for Automobile/Motorvehicle Insurance (For International Moves ONLY). Both you and your shipper/mover are required to sign this form.
	After filling out the required forms kindly email/fax/mail them to our offices. MoveInsure will respond only to forms which are completely filled out and signed.
	Payment can be made either through credit card, check, or a money order made out to MoveInsure Brokerage Ltd. For a cheque allow 6 business days after arrival at our offices for clearance. You can also phone our offices and ask for our bank account details for a direct wire transfer (*) (* a copy of your instructions to your bank should be faxed to our office ++718-7934021).
	Certificate of Insurance will be mailed to the address you specified in the Request for Quotation Form immediately after payment verification.
	Your mover will also be providing you with a "Packing List". The Packing List is not the same as the Cumulative Form/Inventory List we are asking for. The Packing List is required for different reasons. Do not be confused by these two separate lists.
